What is Rebound Hyperglycemia?
Rebound hyperglycemia is the high blood sugar that often follows a low—most commonly because the low was overtreated.
It’s one of the most frustrating patterns in diabetes management: you’re low, you eat quickly (and usually more than you planned), and then an hour later you’re dealing with the opposite problem.
While blood sugar can sometimes rise after a low due to hormonal responses, most of the time in real life, it comes down to how the low was treated. And to be fair—when you feel low, your brain isn’t thinking about precision. It’s thinking: fix this, fast.
With traditional insulin therapy, that approach made sense. Insulin was active in your system regardless of what your glucose was doing, so you needed a meaningful amount of carbs just to counteract it. The “eat first, ask questions later” approach was a reasonable defense.
But with automated insulin delivery, Omnipod 5 is already responding to your glucose in real time by pulling back insulin when it senses a drop, sometimes well before you even feel it. By the time you're reaching for a low blood sugar treatment, the system has often already done some of the heavy lifting.
How Omnipod 5’s SmartAdjust Technology Prevents and Limits Low Blood Sugar
Omnipod 5’s SmartAdjust Technology is continuously working in the background to prevent lows before they happen. Here’s how:
Adaptive Basal Rate
Unlike traditional pumps with fixed basal rates, Omnipod 5 automatically calculates your baseline insulin delivery called the Adaptive Basal Rate and updates it with every Pod change based on your recent total daily insulin needs.
In Automated Mode, SmartAdjust looks 60 minutes into the future and adjusts insulin delivery every 5 minutes. If your glucose is trending down, it reduces or pauses delivery proactively before you’re even technically low.
Micro-Bolus Delivery
Rather than traditional correction boluses, Omnipod 5 delivers insulin in small, frequent micro-boluses. This gives the system more precise, real-time control over how much insulin is active at any given moment to reduce the risk of hypoglycemia (unless the user overrides oopsies).
By the time you see a low alert, insulin delivery has often already been reduced or paused for several minutes.
Treating Low Blood Sugar with Omnipod 5
With SmartAdjust Technology reducing or suspending insulin delivery during a drop, the classic Rule of 15 may simply be too much. What worked before automated insulin delivery can easily become an overtreatment now– and that overshoot is exactly what leads to a rebound high.
The 5–10 Gram Strategy
For most mild to moderate lows in the 55-70 mg/dL range, starting with just 5-10 grams of fast-acting carbs is often all it takes. Because Omnipod 5 has likely already pulled back your insulin delivery by the time you're treating, your glucose doesn't need as much of a push to recover. A smaller, deliberate amount gives your body and the system a chance to do their jobs without overcorrecting.
Practical Tips for Omnipod 5 Users Managing Hypoglycemia
Talk to Your Provider About Your Settings
Frequent lows may indicate your insulin delivery settings may need adjustment.
Target Glucose: This is the only setting you can change that directly impacts how SmartAdjust delivers insulin. If you are running low frequently, your provider may suggest raising it incrementally– even a small bump from 110 mg/dL to 120 or 130 mg/dL can make a meaningful difference. Plus, you can also set higher overnight targets if nighttime lows are a recurring pattern.
Duration of Insulin Action: If your duration of insulin action is set too short, Omnipod 5 underestimates how much insulin is still active and may deliver more than your body needs. This is a common and overlooked driver of lows and worth reviewing if everything else seems dialed in.
Correction Factor: If your correction factor is too aggressive, SmartAdjust may be stacking more corrective micro-boluses than your body actually needs.
Review Your Insulin Delivery History
Before you treat, take five seconds to check what Omnipod 5 has already done– it may change how much you correct your low blood sugar with. In the app or PDM, look for:
Delivery pauses or reductions: If SmartAdjust already paused or significantly reduced insulin in the last 30-60 minutes, you may need a smaller treatment.
Insulin-on-board: Low insulin-on-board alongside a paused delivery is a strong signal your glucose may recover with minimal intervention. High insulin on board means there’s still insulin working and you may need to treat with more.
Carry Measured Treatments
Treating a low doesn’t have to be a guessing game. Pre-measured options like Transcend Minis make it easy to give just enough carbs without overshooting– especially when Control-IQ+ has already slowed or paused insulin.
The Bottom Line
By the time a low alert appears on your screen, Omnipod 5 has likely already been working behind the scenes– reducing or suspending insulin delivery to slow the drop before it gets worse. The system has already done part of the work. That means your treatment doesn't need to be as aggressive as it once was.
In most cases, a small, measured amount of fast-acting carbs, often just 5–10 grams, is enough to bring glucose back into range safely. No need to overcorrect. Trust the system, treat conservatively, and give your body a chance to recover without setting off a rebound high on the other side.
